TSH and Thyroid Blood Test Results Explained

Thyroid results are among the most confusing to read, largely because the main marker works backwards: when your thyroid is underactive, your TSH goes up, not down. This guide explains why, what each marker means, and what typical ranges look like.

Why TSH rises when your thyroid slows down

TSH stands for thyroid-stimulating hormone, and it is not made by your thyroid at all. It is made by your pituitary gland, which monitors your thyroid hormone levels and adjusts its signal accordingly.

If your thyroid is underperforming, your pituitary shouts louder, so TSH rises. If your thyroid is overactive, the pituitary goes quiet, so TSH falls. That inverse relationship is why a high TSH usually indicates an underactive thyroid.

Typical thyroid reference ranges

Ranges vary meaningfully between laboratories, so always compare against the range printed on your own report. As a general guide:

What each marker tells you

TSH is the screening marker and the most sensitive early indicator that something is shifting.

Free T4 is the main hormone your thyroid produces. Read alongside TSH, it helps distinguish a genuine thyroid problem from a temporary blip.

Free T3 is the more active hormone, converted from T4 in the body. It is not always included but can add detail.

Thyroid antibodies indicate whether your immune system is attacking your thyroid, as in Hashimoto’s or Graves’ disease. This is why an autoimmune profile is more informative than TSH alone, and it is included in our thyroid test.

Common patterns and what they suggest

  • High TSH, low T4: typically an underactive thyroid (hypothyroidism). Often causes fatigue, weight gain, feeling cold, dry skin and low mood.
  • Low TSH, high T4: typically an overactive thyroid (hyperthyroidism). Often causes weight loss, palpitations, anxiety, tremor and heat intolerance.
  • Slightly raised TSH, normal T4: sometimes called subclinical hypothyroidism. May need monitoring and a repeat test rather than immediate treatment.
  • Positive antibodies: suggests an autoimmune cause, which affects how the condition is likely to progress and is monitored.

These are patterns, not diagnoses. Interpretation depends on your symptoms, medication and history, which is why a consultation is included with every test we carry out.

Things that can skew a thyroid result

Worth knowing before you test: biotin supplements (common in hair and nail products) can significantly interfere with thyroid assays; recent illness can temporarily alter results; and levels fluctuate through the day, so a morning appointment is often preferred.
